I’m not a data scientist, academic, not an influencer, just a top 1% creator on Linkedin and a social worker by trade. Still, we have:

• a real, organically generated cross-section of care coordinators and clinical-social staff

• from multiple states and care settings

• surfacing consistent, repeatable operational themes

And it revealed something important: That workforce sentiment may serve as an early qualitative signal of operational strain, often surfacing the same pain points that later appear in Stars, utilization, TOC failures, or turnover data.

North Carolina officially launched the Children and Families Specialty Plan today, a single, statewide Medicaid managed-care contract operated by Healthy Blue Care Together for ~31,000 children and young adults in or formerly in foster care. The model consolidates physical, behavioral, I/DD, HCBS, and unmet resource-needs supports into one buyer with full accountability for placement stability and crisis utilization, creating a concentrated, high-acuity SDOH market overnight. Vendors that solve for psych churn, caregiver support, youth housing, and care continuity now have a direct statewide entry point instead of 100+ fragmented county pathways.

One of the roles that stood out to me this week was a Senior Director of Product Marketing at Modivcare. This is a job that looks like “marketing” on paper but is really about something else:

• Unifying payer-facing narratives across Medicaid, MA, and supportive care.

• GTM clarity.

• Pricing logic.

• Margin impact.

• Infrastructure storytelling.

This is exactly what I mean when I say the real SDOH work doesn’t live under “SDOH.”

It lives inside the roles that reshape payer partnerships and build the infrastructure that makes social care billable, measurable, and defensible.

These roles are hard to find, poorly labeled, scattered across industries, and easy to miss unless someone is scanning the market through a strategy lens.

I’d call every shelter. Each one was always full. We’d come up with a plan. But the hospital’s transportation voucher system rarely showed up after five. The cab company didn’t drop off at shelters. What the patient needed was a $20 ride. What I had was a policy binder.

For years, that was where the story of social care ended, in the quiet, bureaucratic places where good people hit the wall of reimbursement logic.
